Individuals Under the Home and Community Based Waiver Program (DDD Services)

What is the HCBS / DDD Waiver Program?
The Home and Community Based Services (HCBS) Waiver Program, administered by the New Jersey Division of Developmental Disabilities (DDD), provides funding and services to eligible individuals with developmental disabilities so they can receive care in community settings rather than institutions.
This program allows individuals to:
- Live at home or in supported community housing
- Participate in meaningful daily activities
- Build independence and life skills
- Remain connected to family, friends, and their community
Our DSP (Direct Support Professional) Services
1st Americare provides individualized, person-centered support delivered by qualified Direct Support Professionals who are trained to meet the unique needs of each individual. Services may include:
Community Inclusion & Companionship
Support with community outings, social activities, volunteering, employment support, and participation in recreational or faith-based activities.
Daily Living & Life Skills Support
Assistance with:
- Cooking and meal preparation
- Housekeeping and laundry
- Money management and budgeting
- Personal organization and routines
- Transportation to appointments and activities
Personal Care & Health Support
Help with:

- Following healthcare plans
Behavioral & Emotional Support
Compassionate guidance for individuals with:
- Autism Spectrum Disorder
- Intellectual Disabilities
- Anxiety, depression, or mood disorders
- Dual diagnosis (mental health and developmental disability)
Person-Centered Planning
We work closely with the individual, their family, and their Support Coordinator to ensure services align with the Individualized Service Plan (ISP). Our approach respects personal preferences, cultural values, and life goals, promoting dignity, choice, and independence.
